Box 2418 Kuala Lumpur However, the 131 I-Rose Bengal faecal excretion test is not only expensive and time-consuming but is often inaccurate because of difficulty in stool collection. The percutaneous liver biopsy is also not without error 4 and consent from parents are not always forthcoming. Some centres have used abdominal scanning for qualitative evidence of isotopic excretion but this is less accurate. 5 Prior to the introduction of our approach in the General Hospital, Kuala Lumpur, evaluation of conjugated hyperbilirubinaemia in infancy involved operative cholangiography and open liver biopsy. This approach will inevitably result in laparotomy of cases with neonatal hepatitis. Duodenal fluid inspection has been adopted in Japan 6.7 and a few centres in USA including South America and has been found to be quite reliable.

Determination of conjugated bile acids in human bile and duodenal fluid by reverse-phase high-performance liquid chromatography.

A simple mehtod using reverse-phase liquid chromatography is presented for resolution and quantitation of the major conjugated bile acids of man, including the glycine and taurine conjugates of the dihydroxy bile acids, chenodeoxycholic and deoxycholic acid. Using modern, high-performance chromatographic equipment, analysis time is less than 30 minutes. The quantitative range of the method, wit...

Endoscopic retrograde cholangiopancreatography in infantile cholestasis.

The difficulty of distinguishing surgically correctable causes of conjugated hyperbilirubinaemia in infants from other causes means that some infants may undergo laparotomy and intraoperative cholangiography unnecessarily, and others may be referred for surgery too late. In an attempt to improve the diagnostic accuracy in infants with conjugated hyperbilirubinaemia when standard methods produce...

Effect of glycine-conjugated bile acids with and without lecithin on water and glucose absorption in perfused human jejunum.

Perfusion studies were performed in healthy volunteers to test whether the secretory effect of conjugated bile acids, previously shown for the colon, was also present in the jejunum. A perfusion system with a proximal occlusive balloon (and continuous aspiration of duodenal secretions) was used; isotonic test solutions contained glycine-conjugated bile acids with or without lecithin. Fluid move...

Effect of conjugated and unconjugated hyperbilirubinaemia on the plasma 25-hydroxy-vitamin D response to ultraviolet radiation in the rat.

1. Vitamin D deficiency was induced in anicteric Wistar rats, Wistar rats before common bile-duct ligation was performed and Gunn rats with unconjugated hyperbilirubinaemia. 2. The effect of ultraviolet radiation on plasma concentrations of 25-hydroxy-vitamin D was studied in a group of control anicteric Wistar rats (n = 11), Wistar rats 8 days after common bile-duct ligation (n = 7) with conju...

Duodenal bile acids in infancy.

Duodenal bile acids have been estimated in three age groups in infancy from birth to 7 months, and age-related changes have been shown. The lowest concentrations of duodenal bile acids occurred in the youngest infants, and increasing concentrations were found with increasing age. Taurine conjugated bile acids predominated in early infancy, while in older infants bile acids were mainly conjugate...
